We know what he did publicly. We have no idea what happened privately.
He was the first coach or coordinator fired mid-season since like the Nixon administration. And we also very rarely fire coordinators in the offseason, instead waiting for contracts to run out (through at least Cowher and Tomlin).
It's pretty clearly a team policy.
You don't have to blame absolutely everything on Tomlin (well...apparently you do, but you shouldn't).
As I said above, I think he gets the blame for hiring Canada.Comment

The Steelers announced the following changes and updates regarding the team:- DC Teryl Austin's position has been elevated to Defensive Mouthpiece
- It is now mandatory that all defensive backs wear yellow gloves to better coordinate with team colors
- Arthur Smith announced new technology enabling "Mute" function on all QBs when attempting to audible out of the playcall
- There will be no more arriving fashionably late to team meetings (Note: G. Pickens exempted)
- A Comfort Therapist will be made available to all players after any regular season losses
- The Defensive Playbook will have one new page added (but, in an effort to reduce miscommunications during games and streamline learning, three previous pages will be deleted)
